Why Hospital Recommendations Don't Always Work at Home
One of the most common things I hear during home safety evaluations is:
"The hospital told us to get this, but it doesn't really work in our home."
Hospital occupational therapists and physical therapists make equipment recommendations with the information available during a hospital stay. These recommendations are often appropriate and necessary to facilitate a safe discharge. However, therapists in the hospital rarely have the opportunity to observe the person's actual home environment.
As a result, equipment that appears appropriate in the hospital may present unexpected challenges once someone returns home.
Why This Happens
Hospitals are designed to be standardized environments. Patient rooms are generally uncluttered, doorways are wide, furniture heights are predictable, and bathrooms are designed to accommodate a variety of mobility needs. However, homes are different.
Every residence has unique layouts, furniture, flooring surfaces, lighting conditions, and daily routines. Small environmental differences can significantly impact safety and independence.
Occupational therapists evaluate not only the person's physical abilities, but also how those abilities interact with their environment. A recommendation that works well in one home may be ineffective—or even unsafe—in another.
Common Examples
The Walker That “Doesn’t Fit”
A walker may work well during a hospital stay, but once someone returns home, they may feel that it is too wide for narrow bathroom doorways, tight hallways, or crowded living spaces.
What many people don't realize is that equipment can often be adjusted to better fit the home environment. For example, front walker wheels can frequently be reversed inward to reduce overall width, and learning techniques such as side-stepping within the walker can help individuals safely navigate tighter spaces.
Without proper education, however, people may assume the walker won't work in their home and begin walking without it, increasing their risk of falls.
OT Tip: Before purchasing new equipment or abandoning existing equipment, have an occupational therapist assess how it functions within your actual living environment. During a home safety evaluation, I often measure key spaces, modify equipment when appropriate, and teach alternative mobility strategies to improve safety and independence at home.
The Tub Transfer Bench That Doesn't Match the Bathroom
Tub transfer benches can be extremely effective for reducing fall risk during bathing. However, not all tubs, bathrooms, and bathroom layouts are the same.
In some homes, limited space around the bathtub may make transfers more difficult than anticipated, requiring modifications to the setup or an alternative recommendation.
OT Tip: I frequently assess bathtub dimensions, surrounding floor space, and transfer mechanics before recommending bathing equipment. Sometimes a tub transfer bench is the safest option; other times a shower chair, grab bars, or a different transfer strategy may be more effective.
The Raised Toilet Seat That Creates New Challenges
Raised toilet seats are frequently recommended after surgery or hospitalization. While they can reduce the amount of bending required, they may also alter transfer mechanics.
For some individuals, particularly those of shorter stature or with balance impairments, a raised seat can actually make it more difficult to safely position their feet on the floor during transfers.
OT Tip: Rather than automatically adding height, I evaluate how the individual transfers on and off the toilet. In some situations, grab bars or an elevated toilet frame provide better support than a raised toilet seat alone.
The Bed That's Much Different Than the Hospital Bed
Hospital beds are adjustable. Most beds at home are not. A bed that is too high or too low can significantly impact an individual's ability to safely transfer in and out of bed, increasing fall risk and caregiver burden.
OT Tip: During home safety evaluations, I often assess bed height and transfer technique. Sometimes a simple bed rail, furniture adjustment, mattress modification, or repositioning of the bed within the room can improve safety and independence.
The Value of a Home Safety Evaluation
A home safety evaluation allows occupational therapists to assess the interaction between the individual, the task, and the environment.
Rather than focusing solely on equipment, occupational therapists evaluate how a person moves throughout their daily routines, including getting in and out of bed, bathing, dressing, preparing meals, and navigating the home safely.
Recommendations can then be tailored to the individual's physical abilities, home layout, and personal goals.
Successful transitions home require more than simply obtaining recommended equipment.
Safety and independence are optimized when recommendations are individualized to the person's environment and daily routines. The right equipment is important—but equally important is ensuring that it works within the home where it will actually be used.
The goal isn't to recommend the most equipment—it's to recommend the right equipment. Sometimes the safest solution is adding a device, and other times it's modifying the environment, changing the setup, or teaching a different strategy altogether.
