Services We Offer
Caregiver Support and Home Safety
Our team help caregivers and families make home a safer place so your loved one can stay independent and avoid injuries. We can support you through:
A home safety assessment to identify fall risks and recommend simple changes or equipment to make daily life easier and safer
A personalized safety plan that is customized to your loved one’s needs, routines, and goals
Caregiver training and education so you feel confident supporting your loved one as they get up, move around, and stay safe at home
Neuro Rehabilitation
Our clinicians help people who have had a stroke, brain injury, Parkinson’s, or other neurological conditions regain skills, move more easily, and stay safe at home. Our treatment may include:
Cognitive strategies to support memory, attention, and thinking during everyday tasks
Safe movement and handling techniques to protect joints and support mobility
Shoulder support and taping to reduce pain, subluxation, and discomfort
Strategies to reduce muscle tightness to make movement and daily tasks easier
Mental imagery and motor-learning exercises to help the brain re-learn movements
Visual Skills Retraining
Changes in vision can make daily life harder – especially after a stroke, brain injury, or eye-related condition. Our clinicians help people improve how they see and move through their day, so tasks feel safer and easier. We support individuals who may experience:
Double vision, blurry vision, or eye strain
Trouble noticing one side of their environment after a stroke or injury (visual neglect)
Low vision or vision loss from macular degeneration or aging
Difficulty seeing contrast, reading text, or navigating unfamiliar spaces
Our goal is to help you see more clearly, stay safe, and feel confident in your daily routines.
Orthotic Fitting
Orthotics (such as splints or braces) can help support the hand, wrist, or arm after injury, surgery, or neurological conditions. Our clinicians provide guidance on choosing and using orthotics so the body stays protected and well positioned throughout the day. What we offer:
Orthotic fabrication and fitting (with a doctor’s order) to support the hand or arm, manage tightness, and maintain proper alignment
Recommendations for positioning devices designed to prevent contracture, reduce swelling, protect skin integrity, and support long-term limb health
Activities of Daily Living
Daily tasks can become challenging after illness, surgery, injury, or changes in mobility. Our clinicians help caregivers and their loved ones learn safe, easier ways to manage everyday routines with confidence. We can support with:
Morning routines – getting dressed, bathing, grooming, or using the bathroom
Getting around safely – getting in and out of a car, walking through the home, or navigating curbs and doorways
Managing medications – organizing pills and keeping track of when and how to take them
Following safety rules after surgery or injury – like hip, heart (sternal), or spine precautions, while still getting daily tasks done
Fall Prevention
Falls are a major concern after illness, injury, or changes in balance and strength. Our clinicians provide targeted balance and mobility training to reduce fall risk, improve confidence, and support long-term independence at home and in the community. Our approach includes:
Individualized balance training to improve stability, strength, and coordination
Customized home exercise programs to maintain progress between sessions
Personalized safety recommendations to stay safe during daily activities – including navigating stairs, uneven surfaces, and community environment
